Job Summary
The Care Practitioner plays a vital role in providing compassionate and high-quality care to individuals in need. This position involves working closely with clients to assess their needs, develop care plans, and implement appropriate interventions., ensuring that the highest standards of care are maintained.

Duties

• Conduct comprehensive assessments of clients' physical, emotional, and social needs.
• Develop and implement individualised care plans in collaboration with clients and their families.
• Monitor clients' progress and adjust care plans as necessary to ensure optimal outcomes.
• Provide direct support and assistance with daily living activities, promoting independence where possible.
• Maintain accurate records of client interactions, care plans, and progress notes in accordance with organisational policies.
• Collaborate with multidisciplinary teams to ensure a holistic approach to client care.
• Participate in ongoing professional development to stay current with best practices in the field.

• Exceptional communication skills, both verbal and written, for effective interaction with clients, families, and colleagues.
• Empathy and compassion for individuals in varying states of health and well-being.
• Strong organisational skills to manage multiple client cases efficiently.

Pay: From £13.00 per hour
Expected hours: No more than 30 per week
